Case Management Program

ESI's Case Management Program is the local implementation of the statewide Case Management Program for the Frail Elderly whose goal is to help Iowa's frail elderly stay independent in their own homes and avoid premature institutionalization.  This program uses a multi-disciplinary approach to providing community-based services and works with Geriatric Care Services in order to provide the highest level of care to clients.

Case management facilitation works as a behind-the-scenes advocate for clients and provides training for case managers and access to special funding, such as Medicaid waivers.  ESI coordinates the case management services in a four county area.

Memory Loss & Alzheimer's Case Manager

Case managers can help families and friends to determine the difference between an individual's simple temporary memory loss and potentially life-threatening memory loss due to dementia or Alzheimer's.  This program is staffed by a registered nurse with specialized training in the areas of aging, memory loss, and dementia.  Case managers can answer questions over the phone or meet with you in your home.  They will explain available services and provide referrals to other community services.  Call 338-0515 to speak with a case manager.

Legal Case Manager

Legal Case Managers will help clients to address legal issues that do not require the assistance of an attorney.  Education and outreach is also available through presentations about various topics including Medicaid eligibility, dependent adult abuse and fraud.  Case managers area available to complete an in-home assessment and coordinate necessary services.  To make an appointment with a legal case manager call 338-0515.

Costs

There is no charge for the Case Management Program or for continued follow-up, however, there may be a charge for required services.  If a client is unable to pay for services, the case manager will work with various agencies to determine a client's eligibility for funding assistance.
