Geriatric Care Services Program (GCS)

This program supports the frail and vulnerable elderly in their effort to remain independently in their own homes.  Program staff can provide answers to both seniors and family caregivers when there are questions about the needs of elders.

Assessment

A GCS team member will meet with an elderly client and possibly their caregiver in the client's home.  If desired, a trained professional will perform an initial assessment using nationally-recognized medical and social model tools and prepare case recommendations based on the assessment and the client's requests. Long-term needs requiring continued follow-up will be coordinated by GCS in conjunction with the case management program in order to ensure the highest quality and most comprehensive care.

Care Plan

Plans are based on the individual needs of the client and all services are supplied by experienced providers and coordinated through GCS.

Eligibility

An individual age 60 and older, who is a resident of either Cedar, Iowa, Johnson, or Washington County is eligible to received services from GCS.  Elders living in other counties and should contact their local area agency on aging for the service provider nearest them.

Costs

At this time, there is no charge for the initial assessment and continued follow-up, however, there may be a charge for services.  Federal, state, and local funds are often available for those unable to pay for services.  Often Elderly Waiver and Senior Living Program funds can be accessed for qualified clients and services.

Clients Come First

While the GCS team member coordinates services, as with all Elder Services programs, client choice is paramount.  Elder Services, Inc. is a Consumer Directed Attendant Care Provider which means that services are provided under the direction of the client.
