Memory Loss Nurse Specialist Program

The Memory Loss Program is staffed by a registered nurse who has specialized training in the areas of aging, memory loss, and dementia.  The nurse provides support to individuals with memory loss or dementia and their caregivers to enable the individual with memory loss or dementia to live safely at home as long as possible.

When does Memory Loss Become a Disease?  Ten Warning Signs

•  Memory loss is affecting normal activities
•  Difficulty performing familiar tasks
•  Problems with language
•  Poor judgement
•  Problems with abstract thinking
•  Misplacing things
•  Changes in mood or behavior
•  Changes in personality
•  Loss of initiative

Direct Services

•  Home visits by a nurse who specializes in working with individuals with memory loss and dementia
•  Complete nursing assessment and follow-up
•  Instruction and education related to memory loss issues
•  Development of individualized plan of action and care\
•  Ongoing client advocacy
•  Client and caregiver support
•  Link to local case management program
•  Link to local home and community-based services
•  Referrals to local medical professionals as needed
•  Family care conferences when needed
•  Information about current diagnosis and treatment options

Additional Services

•  In-service education
•  On-site lending library
•  Specialized training for respite providers

Cost

The cost for this program is covered through grants to Elder Services, Inc.  There is no cost to the client.

Eligibility

Individuals of any age who have issues related to memory loss or dementia, as well as their family members or caregivers, are eligible for services from the Memory Loss Nurse Specialist Program.
