Memory Loss Nurse Specialist Program
The Memory Loss Program is staffed by a registered nurse who has specialized training in the areas of aging, memory loss, and dementia. The nurse provides support to individuals with memory loss or dementia and their caregivers to enable the individual with memory loss or dementia to live safely at home as long as possible.

The cost for this program is covered through grants to Elder Services, Inc. There is no cost to the client.
Eligibility
Individuals of any age who have issues related to memory loss or dementia, as well as their family members or caregivers, are eligible for services from the Memory Loss Nurse Specialist Program.
